Clinical Assessment
Once you decide to move forward, you will have a comprehensive clinical evaluation conducted by a member of our clinical treatment staff. This complete evaluation takes into consideration a complete history of your substance use, any co-occurring mental health conditions, previous treatments, current medications, your support network, and your hopes and goals when it comes to your recovery. The comprehensive clinical assessment will form the basis of everything that follows, and it will be conducted with the same care and dedication that is characteristic of the rest of our program.

Insurance Verification & Payment
There should be no financial barriers preventing you from seeking treatment.
When you come to Forward Motion, our admissions team will confirm your benefits by directly contacting your insurance company and will explain what type of coverage you have, if there are any out-of-pocket obligations, and what options are available to you. We work with most major insurance providers on an out-of-network basis.
For clients who are paying privately, we offer transparent pricing and can help create a payment plan that suits your finances. There are no unexpected costs, hidden fees, or pressure to make a commitment before you understand the full picture.

Treatment Planning
After you have completed your evaluation and assessment, our clinical staff will create a custom treatment plan that reflects who you are as an individual, your specific substance use, any co-occurring mental health disorders, and what type of life you want to live. If you are suffering from dual diagnosis conditions, both your substance use disorder and your mental health disorder will be addressed as one. Your treatment plan is always specific to you, not a one-size-fits-all plan. It is designed for your specific needs and will evolve as you progress and your needs change.

Admission & Orientation
After scheduling your admission, we will walk you through all the things you can expect after you arrive — items you need to bring, typical day activities, who you will be working with, and what you should expect during your first week or two in treatment — because we want you to come into treatment feeling prepared and supported, not anxious about the unknowns. Our staff can also work with you to address any issues related to transportation, family logistics, or other practical matters. Let us know in advance, and we can help.

Discharge & Aftercare Planning
Aftercare planning starts when you first come to treatment. From your initial meeting with our team, we will work with you to establish a plan for life after treatment, including options for step-downs in intensity of care, outpatient treatment options, sober living options, support groups, medication management, and anything else you may need. We believe preparing for life after treatment should be as intentional as the treatment itself.
Who We Admit
Forward Motion Health accepts men who are suffering from substance use disorders (SUD) as well as emotional or behavioral problems and underlying issues that have contributed to their substance use. Our programs can provide the additional intensity needed if regular outpatient treatment services are not enough and you are looking for something more.
Our treatment facility includes a state-of-the-art fitness facility, a dedicated martial arts facility, a cold plunge, and a sauna, so that the men we treat can work toward not just mental wellness, but physical wellness as well.
If we feel that we are not a good fit for your specific circumstance, we will help you find a provider who we believe would be appropriate.

Your privacy will be protected during admissions and through every stage of treatment.
All personal health information is handled in accordance with HIPAA, and the admissions process is conducted with discretion from your first call onward. Your information will never be shared without your consent.
For Families and Referring Providers
We recognize that deciding to go into treatment can involve more than just the patient; partners, social workers, and family members may have a part to play in making that decision as well.
